Risk Assessment Tool
For the Risk Assessment Tool UI, I was tasked with modernizing an existing data visualization dashboard used to identify patients at high risk of readmission or adverse health outcomes within 30 days of discharge. This tool is a widely recognized medical calculator, and within our organization, it is adapted for use in both general healthcare and Behavioral Health assessments. The goal was to create a more intuitive, cohesive design that integrated seamlessly with the scheduling application I had previously worked on. By aligning the new tool’s design with the scheduling app’s interface, I ensured users could transition smoothly between the two without any visual disruption. The redesigned tool maintained the core functionality of the original dashboard, while offering an updated, user-friendly interface that enhanced accessibility and usability for healthcare professionals
The example shown here is the landing page, designed to closely resemble the patient search interface in the scheduling application for a seamless user experience. However, unlike the scheduling tool, this page provides an additional option to proceed without selecting a patient.
This is a key feature, as it allows healthcare professionals to still use the calculator on new patients who have not yet been registered, ensuring that the tool remains useful and accessible even before a patient’s full profile is created. This flexibility is crucial for streamlining workflows and supporting timely decision-making, regardless of patient status.
Landing Zone
This next screen is where the actual calculation takes place. It features multiple input fields that schedulers can use to gather necessary information from patients. Each section of questions is assigned a score, contributing to an overall total score that helps determine the patient's risk. To maintain consistency with the rest of the application, this page uses the company’s vibrant color palette and mirrors the look and feel of the scheduling application.
This cohesive design ensures that users have a familiar and streamlined experience as they navigate through the tool, making it easy to input data and quickly access the calculated results.
